The future value formula is FV = PV(1+r)^n + PMT x [((1+r)^n - 1) / r], where r is the periodic rate and n is the number of periods.
Compound growth means your returns generate their own returns. Over long periods, this compounding effect dominates.
The monthly rate is the annual rate divided by 12.
